Recuperative Nursing Care

At Brighter Health MN, we provide 24-hour access to medical care and supportive services in a safe, accessible environment for individuals experiencing homelessness. Our program includes recuperative nursing care, wound care, medication support, patient education, care coordination, discharge planning, and counseling or peer support, with staff available for wellness checks, secure storage for belongings, three meals per day, and environmental services such as cleaning and laundry. Care is delivered under the supervision of advanced practice providers to ensure safe, high-quality, short-term recovery.

Who We Serve

  • Adults experiencing homelessness
  • Recently discharged from hospitals or clinics
  • Medically stable but unable to recover safely on the street or in a shelter

Eligibility / How to Get Care Who

Qualifies

  • Adults experiencing homelessness or who are unhoused
  • Individuals in need of short-term medical care and support (90 days or less)

Referral Process:

  • Referrals must come from a hospital, clinic, or social service agency
  • Referral forms can be submitted online or via email/fax

Intake Requirements:

  • Completed referral form
  • Medical history and current health information
  • Consent for care and services

Age or Medical Condition Limits:

  • Adults 18 years and older
  • Individuals must be able to safely manage daily activities with support provided in our setting
  • Behavioral health or medical needs beyond what the facility can provide may require alternative care

Facility / Partner Information

Facility Requirements:

  • 24-hour access to a bed and bathroom
  • Three meals per day provided
  • Environmental services such as cleaning and laundry
  • Staff available for wellness checks at least once every 24 hours
  • Secure storage for residents’ belongings
  • Safe, accessible location for the target population

Partnering with BHM

  • Shelters or supportive housing facilities can partner to provide recuperative care services
  • Partners must meet the facility requirements listed above
  • Staff receive guidance and supervision from advanced practice providers to ensure safe, high-quality care
